Our tamper-resistant California Security Prescription forms have the security features specified by the California Board of Pharmacy. You can order individual 1-part prescriptions (S Form) or prescriptions with a carbonless duplicate 2-part (D Form). The new state law, Senate Bill 734 (2006) is phasing out the single drug prescription format. American Security Rx offers R3 Format that allows you to write up to three medications per prescription with a separate set of quantity check off boxes for each medication. The R1 Format will be continued to be offered and it can be used to write more than one medication per prescription. Only one set of quantity check off boxes are offered in this format and more than one box can be checked off depending on the number of medications written. The R1 format provides more space for writing additional information.
American Security Rx offers three prescription sizes: 4.25 x 5.5 inches (S-425 & D-425), 8 x 3.5 inches (S-800 & D-800) and 8.5 X 5.5 inches (S-850 & D-850). Each prescription pad contains 50 prescriptions for the Single form and 50 prescriptions (100 sheets) for the Duplicate form. You can order 500 prescriptions or any multiples thereof (500, 1000, 2500, and 5000). We also offer loose sheet prescriptions for use in ink-jet or laser printers (L Form). The Laser Forms are used with EMR or prescription writing software.
The new state law, Senate Bill 734 (2006), has expanded the use of the Institutional Security prescription forms to clinics that have more than 25 physician or surgeons and to all Hospitals. A designated prescriber will order the prescriptions, receive delivery of, and distribute the prescriptions internally to the licensed health care providers. The individual prescribers will then write or stamp their name along with their DEA and CA license numbers when writing a prescription. The Institutional forms are ideal for clinics and hospitals that have larger turnover of providers such as residency programs or clinics that use per diem providers. The Institutional Laser Forms does not require distribution and record keeping to individual providers since the prescription software program will record the needed information.
